Camp Maas

Tamarack Camps uses two databases to collect the needed camper forms.  The Camper Information Form and Zipline Waiver (Robinson Pioneer only) are collected through CampBrain, the website through which you submitted the camper application.  CampDoc is an online health management system that consolidates and integrates camper health information into a centralized and secure location. These forms must be completed and submitted online by parent/guardian only.

CampBrain (Camper Forms)

Log in to your account using the same username and password you created to submit your camper’s application. At the bottom of the page under “View My Registrations,” click on the green button called “View Registration Details.”

  • The Camper Information Form is located on the right side of the page under the section titled “Forms,” under your camper’s name.  
  • The Zipline Waiver (Robinson Pioneer Only) is located at the bottom of the page under the section titled “Upload Documents.”  Submit the completed waiver (listed as number 2).

If you have more than one camper, be sure you are uploading the form for the correct child.  It says “Documents for (Camper’s Name)” above the various forms.

How to Upload:
Take a picture from your phone or scan the document and click “Upload,” then “Choose File,” select the file from your device, click “Upload,” then “Submit Document.”  


CampDoc (Medical Forms)

Log in will be provided directly from CampDoc via email.  If you are new to the system, watch for the email invitation from CampDoc, including instructions to create your username and password. Be sure to mark them a safe sender.

1. Online Health Profile: Access, complete and submit via CampDoc’s website. 
2. Healthcare Provider Form: Download and print through the CampDoc Online Health Form, completed and signed by camper’s primary healthcare provider, scan and upload directly to CampDoc. We understand that your child’s physical may be scheduled after 4/21, upload as soon as possible. Your child will not be allowed on the bus or permitted to attend camp until the health profile, including the Healthcare Provider Form is completed.
3. Immunization Record: 
The Tamarack Camps immunization policy requires everyone to be fully immunized according to the recommendations of the CDC (Centers for Disease Control), State of Michigan, and AAP (American Academy of Pediatrics). Tamarack Camps full policy can be viewed here. Provide the information by entering the dates directly into the CampDoc system; it will pre-populate from year to year, requiring you only to add any additional immunizations received during the year, and reconfirm the accuracy of the information already listed.
4. Copy: (front and back) of Camper’s Insurance Card: Scan and upload as part of the online health form. Please write your camper’s name and “Camp Maas” on the copy.
5. Pre-packaged Medications: Senior Side Campers ONLY (Grosberg Ruach, Robinson Pioneer, Rosalie and Sol Kaufman Specialty, TSS, and Avodah Villages) Required for these campers taking prescription and over-the-counter medications at camp this summer. Medications will only be accepted through Savon LTC’s pre-packaged system, completed via the link in CampDoc as part of the online health form. Should you choose not to use the pre-packaged system, a $100.00 fee will be assessed, unless you have received prior approval from Roberta Blumberg, Director of Health and Safety.
