2023 Vaccination Clinic

Please complete the following form to register for the Vaccination Clinic on Sunday, April 23 from 1-3 pm at the Tamarack office (6735 Telegraph Rd, Bloomfield Hills, MI 48301).

You may attend anytime between 1 and 3 pm
